Cáncer de mama en Euskadi, a la vanguardia de la investigación


Tras haber alcanzado el ecuador del proyecto de investigación Desiree (Decision Support and Information ManagementSystem for Breast Cancer) y coincidiendo con la revisión del mismo por la Comisión Europea.


Disponemos de las primeras versiones, del sistema de información mas avanzado, a nivel internacional. Posicionando a Euskadi a la vanguardia de la investigación, en el cáncer de mama en el ámbito de las tecnologías de la información.

Centrados en los desarrollos de Bilbomática (Bizkaia), para el manejo de la información heterogénea de las unidades funcionales de mama y donde Fundación Onkologikoa (Gipuzkoa) cuenta con un papel destacado en el ámbito clínico. Se complementa con el líder del proyecto Vicomtech-IK4 (Gipuzkoa), con los Sistemas Avanzados de Soporte a la Decisión.

Para alcanzar el presente grado de avances a nivel internacional, el consorcio se complementa con las aportaciones de nuestros socios:


- Desde la Comunidad Valencia con Eresa y Sistemas Genómicos.

- A nivel Europeo, desde Francia con los hospitales de Saint-Louis y George Pompidou pertenecientes a  al Assistance Publique-Hôpitaux de Paris (AP-HP) y también el centro de investigación INSERM-LIMICS.

- Las empresas Medical Innovation and Technology de Grecia, Arivis AG de Alemania y la Universidad del Ulster de Reino Unido.

Ademas de forma significativa la cooperación internacional se ve reforzada, por la participación de Houston Methodist Research Institute de Estados Unidos, centro de referencia en el cáncer.


A provechando el próximo Día Internacional del Cáncer de Mama (Jueves 19 Octubre), recogemos la rueda de prensa, donde la doctora de Onkologikoa, María Jesús Michelena compareció junto a ella, el consejero de Salud, Jon Darpón, la diputada de Políticas Sociales de Gipuzkoa, Maite Peña, y Txaro Beobide, presidenta de la asociación Katxalin:

"En Euskadi se detectan cada año 1.450 nuevos casos de esta enfermedad. Esta patología se ha incremento en el periodo 2000-2015 a razón de un 1,5% anual."

"Sobre las causas de esta tendencia ascendente ha apuntado al envejecimiento de la población, el estilo de vida, la obesidad, la ausencia de ejercicio físico o factores hereditarios así como al uso de la mamografía que ha permitido diagnosticar más casos cuando son asintomáticos, ha explicado."

"Los datos del Registro de Cáncer sobre la evolución de la supervivencia reflejan que se ha pasado del 83,4% (periodo 2000-2004) al 86% en 2010-2012. Los últimos estudios la sitúan incluso por encima del 90% cuando la enfermedad es detectada en su fase inicial. "

Radio Nervión, entrevista proyecto DESIREE sobre el cáncer de mama

Recogemos una breve entrevista, que nos han realizado en Radio Nervión, donde se recogen los principales objetivos del Proyecto Europeo Desiree,  iniciativa internacional para la mejora del estudio y tratamiento de cáncer de mama.





El proyecto, cuenta con una importante participación española, donde destaca la coordinación por parte del centro tecnológico Vicomtech-IK4, la participación de las empresas Bilbomática, ERESA y Sistemas Genómicos y de la Fundación Onkologikoa, centro con gran especialización en cáncer de mama. El consorcio cuenta además con la participación de grandes centros hospitalarios como Assistance Publique Hopitaux de Paris o el Hospital Metodista de Houston, la Universidad de Ulster (Reino Unido), el centro nacional de investigación biomédica frances INSERM y las SMEs europeas Arivis (Alemania) y Medical Innovation and Technology (Grecia).



La imagen muestra, el sistema de información temporal para la visualización la evolución del paciente y el  Sistema de Soporte a la Decisión para el comité basado en guías clínicas y en la experiencia recogida.

DESIREE: Radiobiological modelling for beginners…

DESIREE BlogExternal radiation or brachytherapy is one of the treatment methods against breast cancer. As all therapies, radiation is linked with side effects. Utmost goal during the treatment with radiation is to maximize the benefit for the patient (efficacy of the treatment) while sparing other vital organs from radiation that could lead to organ’s toxicity.
The assessment of the overall benefit of a radiation treatment can be modelled with the help of linear quadratic model. That model simulates the cell killing for a specific type of cell (each cell cancerous or healthy has different properties: e.g. radiosensitivity, proliferation) under a given radiation prescription and schema.
With the help of such modelling, radiation oncologists can simulate and predict the outcome of any radiation treatment schema. Via treatment simulation, the best radiation treatment plan can be chosen and applied to a given patient.

DESIREE: Modelling the outcome of surgery plus radiation for breast cancer treatment

DESIREE Blog"Breast conserving therapy (BCT) refers to breast conserving surgery (BCS) followed by moderate-dose radiation therapy (RT) to eradicate any microscopic residual disease. The goals of BCT are to provide the survival equivalent of mastectomy, a cosmetically acceptable breast, and a low rate of recurrence in the treated breast.
Studies have shown that, after breast-conserving surgery, radiotherapy to the conserved breast halves the rate at which the disease recurs and reduces the breast cancer death rate by about a sixth. 
These proportional benefits vary little between different groups of women. By contrast, the absolute benefits from radiotherapy vary substantially according to the characteristics of the patient and they can be predicted at the time when treatment decisions need to be made. One of the objectives of DESIREE is the development of a virtual surgery tool that can be applied in clinical practice. The patient will be able to visualise the final aesthetic outcome (cosmetically satisfactory and sensate breast) after breast conservative surgery, while the surgeon will be able to perform a virtual surgery on a three-dimensional model of the breast. The tool will be built on a patient-specific, predictive model of the outcome of BCT that is based on pre-operative imaging of the breast (MRI and 3D surface imaging). Our model combines a mechanical description of the breast tissues with a biological model of the wound healing at the cell level, encompassing multiple scales in space from cells to tissue, and time, from minutes for the tissue mechanics to months for healing. Prediction and simulation of the delivered radiation dose will be performed to quantify and evaluate the radiation treatment plan. Our multi-scale model is being validated with patient data resulting of a clinical trial currently underway ."

Healthinf 2017: Decision Support System for Breast Cancer Committees.

Today we sum up the one day workshop held in the scope of the European project, H2020 DESIREE: Decision Support and Information Management System for Breast Cancer, along with the project leader, Vicomtech-ik4.

Bilbomática is getting ready to participate in the international conference Healthinf 2017 to be held from 21 to 23 February in Oporto (Portugal), where a demonstration of one of the project's work lines will be carried out. This is the new information system that we are developing, which allows the personalized management of patients by Breast Cancer Functional Units.

The new system called DESIMS allows users to sort and manage all the clinical information that is generated in the hospital services where the patient is treated: Radio diagnosis, Nuclear Medicine, General Surgery, Anatomy, Pathology, Radiotherapy, Oncology, Rehabilitation, Psychology, Surgery Plastic, etc. is integrated with the
Genetic Information Systems, with the advanced medical imaging systems and the Decision Support Toolkit.


Building this platform oriented to personalized medicine based on the patient is a major global challenge. The system must allow professionals to work with clinical information, geneticinformation, advanced medical images and to have a Decision Support Toolkit (CDSS) based on three pillars: clinical guidelines, experience and similar cases.

The purpose of the International Conference on Health Informatics is to bring together researchers and practitioners interested in the application of information and communication technologies (ICT) to healthcare and medicine in general and to the support of persons with special needs in particular.
Databases, networking, graphical interfaces, data mining, machine learning, intelligent decision support systems and specialized programming languages are just a few of the technologies and research areas currently contributing to medical informatics. Mobility and ubiquity in healthcare systems, physiological and behavioral modeling, standardization of technologies and procedures, certification, privacy and security are some of the issues that medical informatics professionals and the ICT industry and research community in general are addressing in order to further promote ICT in healthcare. In the case of medical rehabilitation and assistive technology, research in and applications of ICT have contributed greatly to the enhancement of quality of life and full integration of all citizens into society.


European Project Space
The European Project Space (EPS) provides an opportunity for researchers and practitioners involved in ongoing or past European research projects to present the objectives and outcomes of these projects as well as future plans regarding continuation opportunities. The presentation of demos, posters and panels at the same venue and simultaneously with several related conferences allow a large audience among conference participants to get a better insight into the research projects that participate in this initiative. Additionally, representatives of the European Commission will address the audience and present new research projects financing opportunities related to Horizon 2020.

The EPS that will be held in Porto is related to biomedical engineering systems and technologies, involving a related conference namely BIOSTEC, to provide insights into cutting edge research work and R&D projects, originating from Academia or Industry, either in Europe or elsewhere. This initiative intends to create opportunities for knowledge and technology sharing, and establish the basis for future collaboration networks involving current project partners and interested conference delegates."

BIOSTEC is a unique forum for universities, research groups and research projects to present their research and scientific results, be it by presenting a paper, hosting a tutorial or instructional course or demonstrating its research products in demo sessions, by contributing towards panels and discussions in the event's field of interest or by presenting their project, be it by setting up an exhibition booth, by being profiled in the event's web presence or printed materials or by suggesting keynote speakers or specific thematic sessions.

Special conditions are also available for Research Projects which wish to hold meetings at INSTICC events.

The aim of ANDREA is to develop an Active Nanocoated DRy-electrode for Eeg Applications.Multichannel electroencephalography (EEG) is a well-established method for investigating the function of the human brain. However, despite continuous advancements in signal amplification and data processing, difficult and error-prone signal acquisition on the head surface is still a major issue limiting its employment in basic and clinical research.

NIW will investigate possibilities for the integrated and interchangeable use of the haptic and auditory modality in floor interfaces, and for the synergy of perception and action in capturing and guiding human walking. Its objective is to provide closed-loop interaction paradigms, negotiated with users and validated through experiments, enabling the transfer of skills that have been previously learned in everyday tasks associated to walking, and where multisensory feedback and sensory substitution can be exploited to create unitary multimodal percepts.

The DESIREE project aims to provide a web-based software ecosystem for the personalized, collaborative and multidisciplinary management of Primary Breast Cancer by specialized Breast Units (BU), from diagnosis to therapy and follow-up. For that DESIREE provides decision support on the available therapy options by incorporating experience from previous cases and outcomes, and thus, going beyond the limitations of existing guideline-based decision support systems (DSS).  
Specific interactive tools will be also developed for quantitatively analysing medical images and fusing (registering) different imaging modalities with complementary information for enhanced insight. The system will be also connected to a genomic and bioinformatics platform leveraging NGS data offered by Sistemas Genomicos (SIG) that will help personalized diagnostic and treatment and provide valuable information about predictive value in the outcomes of some treatments, based on the patient individual characteristics.
Another objective of DESIREE is to develop a virtual surgery tool that could be applied in clinical practice, based on a multi-scale physiological model that predicts the outcome of breast conservative therapy (BCT). The model will have important physical and psychological implications for the patient regarding the outcome of conservative surgery and will enhance the patient-physician interaction when choosing the treatment.
Finally, the heterogeneous data and knowledge bases, the DSS and the analysis and modelling tools will be integrated into a secure web-based software infrastructure. This web-based software will be provided with highly visual interfaces for exploring the patient case. It will bring the necessary features for enhancing the coordination and management of different patient cases in the BUs and for exploring all the accumulated heterogeneous multi-scale information from previous patient cases in an intuitive manner.

Healthinf 2017 : Sistema de ayuda a la toma de decisión en los Comités de Cáncer de Mama.

Hoy recogemos la jornada de trabajo en el ámbito del proyecto Europeo, H2020 DESIREE: Decision Support and Information Management System for Breast Cancer, junto al líder del proyecto Vicomtech-ik4.

Bilbomática, se encuentra preparando la participación en la conferencia internacional Healthinf 2017 que se celebra del 21 al 23 de Febrero en Oporto , donde se va a realizar una demostración de una de las lineas de trabajo del proyecto, que es el  nuevo sistema de información que nos encontramos desarrollando, que permite a las Unidades Funcionales de Cáncer de Mama, la gestión personalizada de las pacientes. 

El nuevo sistema denominado DESIMS, permite disponer y gestionar toda la información clínica que se genera en los servicios del hospital donde se trata a la paciente: Radiodiagnóstico,  Medicina Nuclear,  Cirugía General, Anatomía Patológica,  Radioterapia,  Oncología,  Rehabilitación,  Psicología,  Cirugía Plástica , etc se integra con los Sistemas de información genética ,  con los Sistemas de imagen medica avanzados y con la Plataforma de Ayuda a la Toma de Decisión.

Siendo un reto de primera magnitud a nivel mundial la construcción de esta plataforma orientada a la medicina personalizada basada en la paciente, donde se permite a los profesionales trabajar con información clínica, con información genética, con imagen medica avanzada y disponer de una plataforma de Ayuda a la Toma de Decisión (CDSS) basada en tres pilares: Guías clínicas , en la experiencia y en los casos similares.
